29 CFR 1910.146(f)(10): The entry permit that documented compliance and authorized entry to a permit space did not identify the results of initial and periodic tests performed under 29 CFR 1910.146(d)(5), accompanied by the names or initials of the tester and by an indication of when the tests were performed:    Before entry was authorized to enter the indoor surge tank, the employer did not document the completion of measures required by preparing an entry permit to ensure that the initial and periodic air monitoring test results were written down by the entry team.  The entrant was exposed to potential health hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts:  On or about September 1, 2015 an employee engaged in unjamming boxes on the line 1 box erector was exposed to struck by hazards in that the moving cam was not guarded at panel door B of the machine.
