29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):     a) Production Floor: The Needle Punch # 2 machine did not have an enclosure that was interlocked to prevent an employee exposure to an amputation hazard from a chain and sprocket. On or about 9/12/2015, an employee sustained an amputation to his left middle finger while operating the Needle Punch # 2 machine near the point of operation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points and rotating parts:      a) Non-Woven Area: The employer did not ensure that employees are protected from amputation hazards through machine guarding of rollers and other rotating parts. On 10/23/2015 an employee sustained a bone fracture while working on the unguarded roller on the High Loft # 3 machine.        Precision Custom Coating was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 942873, citation number 3, item number 1 and was affirmed as a final order on 4/16/2014, with respect to a workplace located at 200 Maltese Drive, Totowa, New Jersey 07512.    Precision Custom Coating was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 314680570, citation number 1, item number 1 and was affirmed as a final order on 1/12/2012, with respect to a workplace located at 200 Maltese Drive, Totowa, New Jersey 07512.
