29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail required by the regulation:   A)    Ventra Sandusky, LLC, located at 3020 Tiffin Avenue, Sandusky, OH 44870: On or about July 17, 2015, the employer did not provide the required injury or illness description on the OSHA 300 Log for the calendar year 2015. The descriptions in Column F, case numbers 85, lacked the required information including, but not limited to, correct injury or illness of the affected part of the body and the object/substance that caused the injury.     B)    Ventra Sandusky, LLC, located at 3020 Tiffin Avenue, Sandusky, OH 44870: On or about July 28, 2015, the employer did not provide the required injury or illness description on the OSHA 300 Log for the calendar year 2015. The descriptions in Column F, case numbers 92, lacked the required information including, but not limited to, the object/substance that caused the injury.     C)    Ventra Sandusky, LLC, located at 3020 Tiffin Avenue, Sandusky, OH 44870: On or about August 26, 2015, the employer did not provide the required injury or illness description on the OSHA 300 Log for the calendar year 2015. The descriptions in Column F, case numbers 109, lacked the required information including, but not limited to, the object/substance that caused the injury.
