29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:   Warehouse; on or about 07 October 2015 and times prior thereto; the employer did not ensure that emergency exit lighting was available. This condition exposed employees to delays in exiting the building and smoke inhalation, fire, and tripping hazards.

Welding Bay; on or about 07 October 2015 and times prior thereto; A knockout plug was missing from a 240 volt energized electrical receptacle used to provide power to the Miller Welder. This condition exposed employees to electrical shock and electrical burn hazard.

Building 618B Fuel Distribution; on or about 07 October 2015 and times prior thereto; a 25 foot energized extension cord was used as permanent wiring and routed through a wall opening. This condition exposed employees to electrical shock and electrical burn hazard.
