Middle Warehouse/Production Office - On or about October 13, 2015, the staircase leading to the second floor level with stairs measuring less than forty four (44) inches wide - was not equipped with a complete guard railing system (missing mid rails) on the open side. Employees were thereby exposed to falls hazards.
