29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a. At plant 8 on the column extruding machine (line 3), employees were not protected from point of operation hazards of the cutter on or before October 14, 2015. An employee suffered an amputation of the thumb when attempting to push the column against the fence.

29 CFR 1904.7(b)(3): How do I record a work-related injury or illness that results in days away from work? When an injury or illness involves one or more days away from work, you must record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known.    For the calendar year 2015 OSHA 300 log, the employer had not recorded the Away From Work (days) for employees who were injured and had not returned.    a. Case No. 9 was marked with "Days away from Work" and no "Away From Work (days)" was entered.    a. Case No. 11 was marked with "Days away from Work" and no "Away From Work (days)" was entered.

29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.    For the calendar year 2015 OSHA 300 log, the employer had not included all the information in column F as required for the following entries:    a. Case no. 5 did not include a description of the injury and which hand was affected.    b. Case no. 7 did not include  a description of the injury and the body part that was affected.    c. Case no. 9 did not include a description of the injury and the body part that was affected.    d. Case no. 11 did not include a description of the injury.
