29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    On or about October 23, 2015 two employees installing the frame on a sign were operating an aerial boom lift and were not wearing any type of fall protection. This condition exposed employee to falls exceeding 10 feet.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    On or about October 23, 2015, employer did not instruct employee to recognize the hazards of working on an aerial boom lift without fall protection equipment. This condition exposed employee to fall hazards exceeding 10 feet.
