29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  On or about November 10, 2015- job site at 186-05 Radnor Road, Jamaica, NY 11432  the employer permitted employees to work in the vicinity of an unguarded floor hole, approximately 11 feet above the   next lower level.   WRITTEN ABATEMENT CERTIFICATION IS NOT REQUIRED.

WRITTEN ABATEMENT CERTIFICATION IS NOT REQUIRED.
