29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  a) On or about November 18, 2015 and times prior; Model CS-14ML, S/N 0703-03, crosscut pop-up table saw(blue): The employer failed to ensure the manufactures installed hood guard with attached hand guards was properly maintained in that the forward hand guard had broken off resulting in an employee pushing a wood strip with his hand and making contact with the blade causing a finger tip amputation.
