29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) Stacker stick feed conveyor catwalk: On or about January 19, 2021: the employer exposed employees to fall hazards of approximately 13 feet by failing to ensure that the edges of the stick feed conveyor catwalk was protected along its entire length on both sides, at the end of the catwalk there were two openings one 26" x 13" and one 18" x 13" which were not protected by a guard rail system or the conveyor system.

29 CFR  1910.147(d)(2):The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:  (a) Roller conveyor between stacker and dip tank: On or about January 13, 2021 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were allowed to work on the stacker outfeed conveyor to adjust/repair the drive chain without shutting it down and locking it out in accordance with the company's Lockout program established procedures.

29 CFR 1910.265(c)(18)(i): Construction, operation, and maintenance of conveyors was not in accordance with American National Standard B20.1-1957, which is incorporated by reference as specified in Sec. 1910.6:   Section 602 Pulleys, Sprockets, Sheaves, Drums, Blocks:  (a) Roller conveyor between stacker and dip tank: On or about January 13, 2021 and at times prior thereto, the employer exposed employees to caught-in and amputation hazards in that the employer permitted employees to work near an unguarded chain and sprocket powering the roller conveyor.

29 CFR  1910.305(b)(1)(i):Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:  a) Dip tank hydraulic lockout point; On or about January 19, 2021: the employer exposed employees to electrical hazards in that employees were allowed to walk and work around electrical conductors entering an electrical disconnect box was not protected from abrasion.
