29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:   a) 552 West Union Avenue, Bound Brook, NJ 08805. Employees were operating pneumatic nail guns, installing new framing materials while not wearing any eye and or face protection exposing them to eye and face injuries from flying debris.     Condition occurred on or about 11/18/15.

29 CFR 1926.404(b)(1)(iii)(c): Cord sets, attachment caps, plugs, receptacle of cord sets and any equipment connected by cord and plug was not visually inspected before each day for external defects such as deformed or missing pins or insulation damage and for indications of possible internal damage. Damaged or defective equipment was not removed until repaired.   a) 552 West Union Avenue, Bound Brook, NJ 08805. Employee was cutting plywood sheathing using a circular saw when the saw had not been inspected for external defects in that the manufactures cord had been replaced with a section of extension cord which was also damaged exposing the employee to electrical shock.     Condition occurred on or about 11/18/15.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) 552 West Union Avenue, Bound Brook, NJ 08805. Employees were installing new sill plates on a second floor deck with no fall protection systems in place exposing them to fall hazards.     Condition occurred on or about 11/18/15.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) 552 West Union Avenue, Bound Brook, NJ 08805. Employees were using a portable aluminum ladder to access the second floor deck when the ladder did not extend three feet above the landing exposing the employees to fall hazards.  Condition occurred on or about 11/18/15.
