Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to hazards associated with falls while stepping/walking on a belt conveyor: a) On or about December 4, 2015 and at times prior, employees were exposed to falls while stepping/walking on the belt conveyor to walk between the entrance and beginning of the conveyor to the work desk to load boxes.

29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  Back exit; Cesyl Mills, 95 West Main Street, Millbury, Massachusetts; The employer did not ensure that this exit route was unobstructed or that stairs or ramps were provided where the exit discharge  route was not substantially level.    a. The exit in the back of the building was locked and had a significant drop of over two feet to the ground below with no stairs or platform. b. Boxes were stacked in the aisle in the loft.
