29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about December 15, 2015, the employer failed to effectively guard the guideline pulley system which is used to pull out large pieces of metal after they have been processed from the #1 Corrugator machine (typically the metal tube measures 15 inches in diameter and 80+ feet in length).  The guideline pulley system is motorized and comprised of a nylon flat sling with a nylon rope tied around it and at the end of this sling is a carabineer connected to a pair of vice grips.
