29 CFR 1910.95(i)(5): The employer did not ensure proper initial fitting and supervise the correct use of all hearing protectors:    a) Chicago Pallet Service, Inc. did not ensure all employees were properly fitted and correctly wore the Howard Leight Max foam ear plugs.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
