29 CFR 1910.147(a)(2)(ii)(B): Where an employee was required to place any part of his or her body into an area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle, lockout tag out was not performed:    (a)  KM16 SAW ROOM - On or before December 21, 2015 employees operated the KM16 5(five) head saw, and while operating the saw, lumber would become jammed in between the saws.  Employees climbed over the front of the saw chassis or walked behind the saw chassis, and cleared the jam by using a board to dislodge the wood that had jammed the saw.  While unjamming the saw, the employees were in between or close to the saw blades, and the saw were not been de-energized and locked and tagged out of service, thereby exposing the employees to amputation injuries.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.213(f)(1): Feed rolls and saws on self-feed circular saw(s) were not protected by a hood or guard to prevent the hands of the operator from coming in contact with the in-running rolls or saws:    (a)  KM16 SAW ROOM - On or before December 21, 2015, employees operate the KM16 5(five) head saw, and while operating the saw, lumber would become jammed in between the saws.  Employees climbed over the front of the saw chassis or walked behind the saw chassis, and cleared the jam by using a board to dislodge the wood that had jammed the saw.  While unjamming the saw, the employees are in between or close to the saw blades, and the hood installed on each saw did not fully guard the saw blade.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:    (a)   KM16 SAW ROOM, LOWER PRODUCTION SHOP - Employees working in the lower production shop used the passageway underneath the Unscrambler table to travel to and from the front and rear side of the KM16 Saw/Band Saw machines.  The chain and sprocket drive of the Unscrambler table did not contain a guard on the rear/underside of the drive chain and pulley, which thereby exposed those employees to caught in hazards, on or about December 21, 2015.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
