29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    JOB SITE: 4650 Lansdowne Avenue, Saint Louis, MO 63116, on or about 1/20/2016, on or about 1/20/2016, at this facility located in the warehouse & receiving area, three designated exit routes were obstructed with the following:    1)    In receiving area on the northeast wall, an exit route was being obstructed with a forklift and other material stored up against the door.    2)    In storage area, on the southeast wall, an exit route was being obstructed with a large 4 X 8 box resting directly up against the door.    3)    In the cushion storage area, on the north wall, an exit route was being obstructed by the large cushions and other material.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    JOB SITE: 4650 Lansdowne Avenue, Saint Louis, MO 63116, on or about 1/20/2016, located in the warehouse receiving dock on the southwest wall of the door way, the wiring from the portable lamp was pulling away from the terminal ends.  The workers were exposed to an electrical shock, burn or electrocution hazard.
