29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling:  On or about January 26, 2016 at a work site located at 2908 Medina Road, Medina, OH 44256, the employer did not ensure the front edge of the scaffold platform was not 14 inches or less from the face of the work. The fabricated frame scaffold was erected inside the building and the employees were nailing band boards on the outside which was approximately 20-24 inches away from the front edge of the platform.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about January 26, 2016 at a work site located at 2908 Medina Road, Medina, OH 44256, the employer did not ensure employees on a fabricated frame scaffold were protected from fall hazards approximately 12-13 feet by a guardrail system. The employees were nailing band boards on the outside which was approximately 20-24 inches away from the front edge of the platform.

29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement:  On or about January 26, 2016 at a work site located at 2908 Medina Road, Medina, OH 44256, the employer did not ensure the footings for the fabricated frame scaffold were sound, level and rigid. The fabricated frame scaffold was erected on a gravel parking lot and on wheels.
