29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about July 11, 2018 at a worksite located at 4000 Ramseyer Lane, Wooster, OH 44691, during commercial framing activities, the employer did not ensure employees were protected from impact hazards by ensuring head protection was available and worn while working underneath employees working in the trusses up to 13.8 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On or about July 11, 2018 at a worksite located at 4000 Ramseyer Lane, Wooster, OH 44691, during commercial framing activities, employees were exposed to fall hazards of approximately 13.8 feet. The employer failed to ensure fall protection systems/devices were used by all employees working in the pre-engineered wooden trusses.
