29 CFR 1910.176(b): Bags, containers, bundles., etc, stored in tiers were not stacked, blocked, interlocked, or limited in height so that they were stable and secured against sliding or collapsing. a) Jobsite: Bale Storage Area by Loading Dock - The employer did not ensure the bales of recycled paper material were securely stacked exposing employees to falling object hazards. A 2000 lb. bale stacked approximately 8 feet from ground level fell and struck an employee causing fatal injuries. Noted on or about 3/1/16.
