29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: Guardrail systems; Safety net systems; or Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. a) Jobsite: 5201 Bleigh Ave, Philadelphia, PA 19136 - The Employer did not ensure an employee traversing through the structure of the elevated B115 and B305 conveyor systems to conduct preventative maintenance on the conveyors, up to approximately 13 feet above the main floor, was protected from falling over the unprotected sides and edges of the walking-working surfaces of the structure of the conveyor systems by the use of a fall protection system. Condition noted on or about January 31, 2023.

29 CFR 1910.140(c)(13)(ii): Designed, installed, and used, under the supervision of qualified person, as part of a complete personal fall protection system that maintains a safety factor of at least two. a) Jobsite: 5201 Bleigh Ave, Philadelphia, PA 19136 - The employer did not ensure the structural members of the elevated B115 and B305 conveyor systems used by employees as the anchorages for personal fall protection systems were designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall protection system that maintained a safety factor of at least two. Employees conducting preventative maintenance on the elevated conveyor systems at heights up to 13 feet above the main floor used the structural members of the conveyor systems as the anchorages for their personal fall protection system when working from walking-working surfaces with unprotected sides and edges. Condition noted on or about January 31, 2023
