Carey, Ohio; On or about March 10, 2016 the employer failed to ensure employees were protected against pinch point hazards created between the action pins and sheets of metal on the U.S. Industrial Machinery Shear located in the fab shop. Employees were exposed to amputation and crush hazards.

Power Press located in the fab shop was enclosed to protect employees from hazards created by the rotating parts and nip-points.

Carey, Ohio; On or about March 14, 2016 the employer failed to ensure that employees who operate the Towmotor Forklift in the small fab shop completed formal training. Employees operate the Towmotor forklift to move sheet metal and materials throughout the shop.
