29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) 203 Greenacre Ave., Longmeadow, MA: On or about March 17, 2016, employees were using a ladder to access the roof of this residence that did not extend the required 3 feet past the upper landing surface.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:    a) 203 Greenacre Ave., Longmeadow, MA: On or about March 17, 2016, employees were roofing this residence more than 9 feet above the ground below without using any fall protection.     Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
