29 CFR 1910.178(m)(3): Unauthorized personnel were permitted to ride on powered industrial truck and/or a safe place to ride was not provided where riding of trucks was authorized: On or about March 16, 2016, the employer is failing to protect employees engaged in forklift operation activities inside the shop and outside the yard. This was most recently documented where employees were exposed to struck-by hazards in that the helper was allowed to ride on the Sellick #1612, Model S120 4WD, Serial 6706607S122-4 Rough Terrain Forklift while a safe place to ride was not provided. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
