29 CFR 1910.178(m)(3): Unauthorized personnel were permitted to ride on powered industrial truck and/or a safe place to ride was not provided where riding of trucks was authorized:  On or about March 16, 2016, the employer is failing to protect employees engaged in forklift operation activities inside the shop and outside the yard. This was most recently documented where employees were exposed to struck-by hazards in that the helper was allowed to ride on the Sellick #1612, Model S120 4WD, Serial 6706607S122-4 Rough Terrain Forklift while a safe place to ride was not provided.  29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.

29 CFR 1904.29(a): 29 CFR 1904.29(a):  The employer did not use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report:  The employer did not use OSHA 300 (Log of Work-Related Injuries and Illnesses) forms, and the equivalent forms were not available for review.  29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
