29 CFR 1910.178(m)(5)(iii): When the operator of an industrial truck is dismounted and within 25 ft. of the truck still in his view, the load engaging means must be fully lowered, controls neutralized, and the brakes set to prevent movement.    a)  On  April 14,2016 at CHEP Recycled Pallet Solutions, LLC 1501 Southern Drive STE D, Kansas City MO: At the Front Dumpster Area: A forklift operator was exposed to struck by hazards, as the forklift operator did not lowered the forks of the forklift when exiting the forklift.  The forklift driver exited the forklift while it was running empty the forklift trash hopper, as the dumping mechanism was broken.

29 CFR 1910.212(a)(1): One or more methods of machine guarding must be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     a) At CHEP Recycled Pallet Solutions 1501 Southern Drive Kansas City MO: On the paint line through to the fork lift pick area: The employer failed to guard the conveyor line exposing employees to caught in, crush by and pinch point hazards by not providing one or more methods of guarding rotating shafts, in going nip points. Employees were allowed to move stacks of pallets manually and to jump into the unguarded conveyor line. On April 7, 2016, an employee received a shoulder and arm injuries when working near an unguarded conveyor line.       Abatement certification for this violation is required.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains must be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided.     A.)  CHEP Recycled Pallet Solutions, 1501 Southern Rd, STE B, Kansas City MO; The employer failed to provide guards for the sprockets and chains of the conveyor line from the paint turn table, to the stencil turn table. Employees were exposed to caught in hazards, on the paint line, dry line and stencil line.  Sprocket wheels and chains which were not enclosed. On April 7, 2016, an employee suffered a shoulder and an arm injuries while working near the unguarded conveyor line.    Abatement for this violation is required.
