29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors:   On or about May 03, 2016 and at times prior thereto, employees were working at Cactus drilling Rig #148. The mud loggers' trailer had a 480 volt cable that ran through a hole at the metal base of the trailer. This condition exposed employees to electrical shock hazards.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling:  On or about May 02, 2016 and at times prior thereto, employees were working at Cactus Drilling Rig #148. Several extension cords were connected together (daisy-chained) to power the sewage pump from behind the Tool-pusher trailer. The employees were exposed to a fire and electric shock hazards.
