TransGlobal, Inc. located at 5271 SR 199 Carey, OH; On or about May 25, 2016, the employer did not ensure employees were protected while performing hand in die operations on the 80T Johnson Machine and Press Corp. part revolution mechanical power press. Employees are required to operate the press without a brake monitor or anti-repeat function and are exposed to amputation hazards in that the press safety features are not control reliable.

TransGlobal, Inc. located at 5271 SR 199 Carey, OH; On or about May 25, 2016, the employer did not ensure inspections were completed on the 80T Johnson Machine and Press Corp. part revolution mechanical power press. Employees are required to operate the press periodically and are exposed to crush hazards from press malfunctions that would otherwise be found during inspections.
