29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems: a) At 830 US Hwy 98, Daphne, AL 36526: On or about June 3, 2016 and at times prior, the employer exposed employees to fall hazards in that employees were allowed to work installing roof truss' at heights greater than 30 feet without being protected from falling by a guardrail system, safety net system or personal fall arrest system. b) At 830 US Hwy 98, Daphne, AL 36526: On or about June 3, 2016 and at times prior, the employer exposed employees to fall hazards in that employees were allowed to access a ladder from the 2nd floor to the 3rd floor that was located on a balcony that had unprotected edges and exposed employees to falls of greater than 10 foot to the ground below.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l). a) At 830 US Hwy 98, Daphne, AL 36526: On or about June 3, 2016 and at times prior, the employer exposed employees to struck-by and crushed-by hazards in that employees were allowed to operate the JLG Skytrak 10054 powered industrial truck without first being trained and evaluated.
