29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.   a) Jobsite: 2nd floor: Employees conducting framing operations around a floor hole were exposed to fall hazards of approximately 8 feet as the hole was not covered or protected with an adequate guardrail system.  Observed on or about 6/10/16.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section.  a) Jobsite : 3rd and 4th level - Employees conducting framing activities were exposed to fall hazards while working along open and unprotected sides and edges of a 4 story residential construction building, approximately 27 and 36 feet above ground level, without the use of a fall protection system.  Observed on or about 6/10/16.  American Dream Carpentry, Inc.  was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1122641, citation number 1, item number 1a, and was affirmed as a final order on 4/29/16, with respect to a workplace located at125 S Van Pelt Street, Philadelphia, PA 19103.
