OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards:      (a) Sherwood Enterprises, Inc. at the Ryan 14X-09 Wellsite: On or about June 18, 2016, employees were engaged in a rig-assist snubbing operation where well control was not maintained and Sherwood Enterprises failed to exercise stop work authority before the snubbing crew exposed a hole in the last joint of pipe, resulting in the release of well gases and fluids into the work area at the Ryan 14X-09 wellsite.    Abatement Note:    Among other methods, one feasible and acceptable abatement method would be to follow API RP 54 (R2013), Section 6.1.1 and maintain well control at all times. One means of controlling a pressurized well is a process called "killing the well". Considerations shall be made to ensure appropriate equipment and materials are on location and operational before work commences.
