29 CFR 1926.416(a)(1): Employee(s) were permitted to work in proximity to electric power circuits that were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  a) 8045 Frankford Ave, Philadelphia, PA - The employees were permitted to work in close proximity to energized power lines that were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means. This hazard exposed the employees to shock/electrocution hazards. This condition was observed on or about July 25, 2016.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system:  a) 8045 Frankford Ave, Philadelphia, PA - An employee working approximately 15 feet above the ground was not using a fall protection system thus exposing the employee to fall hazards. This condition was observed on or about July 25, 2016.
