This condition exposed employee to electrical shock hazards from contact with the 120 volt circuit.  (b)  Madison Rose, Forward Deck Area:  On or about July 27, 2016 and at times prior thereto, strain relief was not provided for the attachment plug on the Miller XMT 350 CC/CV welder.  This condition exposed employee to electrical shock hazards from contact with the 240 volt circuit.
