Additionally, an unapproved job made white plastic cover that installed to adapt the box from quad receptacle to dual receptacle.        e) Builder/Repair Station 5:  On or about August 2, 2016, employees were exposed to potential electrical shock due to a metal electrical outlet box missing knockout covers, exposing the live conductors.     f) Builder/Repair Station 6:  On or about August 2, 2016, employees were exposed to potential electrical shock due to a metal electrical outlet box missing knockout covers, exposing the live conductors.     g) Builder/Repair Station 7:  On or about August 2, 2016, employees were exposed to potential electrical shock due to a metal electrical outlet box missing knockout covers, exposing the live conductors .   Additionally, the conduit was pulled away at its connection exposing the conductors entering the box to sharp edges. An unapproved job made white plastic cover was installed to adapt the box from quad receptacle to dual receptacle.        h) Builder/Repair Station 8:  On or about August 2, 2016, employees were exposed to potential electrical shock due to a metal electrical outlet box missing knockout covers, exposing the live conductors.  Additionally, an unapproved job made white plastic cover was installed to adapt the box from quad receptacle to dual receptacle.      i) Builder/Repair Station 9:  On or about August 2, 2016, employees were exposed to potential electrical shock due to a metal electrical outlet box missing knockout covers, exposing the live conductors.  The faceplate/cover had been installed pinching and exposing an energized conductor.  Additionally, an unapproved job made metallic cover was installed to adapt the box from quad receptacle to dual receptacle.       j) Builder/Repair Station 10:  On or about August 2, 2016, employees were exposed to potential electrical shock due to a metal electrical quad outlet box missing 1 set of receptacles and had no hole cover, exposing the live conductors.     k) Builder/Repair Station 11:  On or about August 2, 2016, employees were exposed to potential electrical shock due to a metal electrical outlet box that did not have the cover/faceplate properly installed, leaving a large gap and exposing the live conductors.     l) Builder/Repair Station 12:  On or about August 2, 2016, employees were exposed to potential electrical shock due to a metal electrical outlet box missing knockout covers, exposing the live conductors.  Additionally, an unapproved job made white plastic cover that installed to adapt the box from quad receptacle to dual receptacle.      NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
