29 CFR 1904.39(a): The employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident:     a. 805 Satellite Blvd, Suwanee, GA - The employer failed to notify the Atlanta-East OSHA office of an employee's amputation and in-hospitalization.  The employee amputated his finger while operating a table saw and hospitalized on August 5, 2015 and was not reported to the OSHA Area Office until August 9, 2016.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   a.  805 Satellite Blvd, Suwanee, GA (Throughout the shop) - Employees were exposed to hazard of sudden energization of moving parts of equipment.  Employees (maintenance) perform servicing and maintenance on various machinery and equipment such as, but not limited to the CNC  machines and production slitting machines.  An internal audit was not performed to ensure de-energization procedures were being properly implemented, on or about 08/15/16.
