29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  a) At Lot 180 Stone Park, Pike Road, Alabama:  On or about August 16, 2016 and at times prior; the employer allowed his employee to work from a 9 inch walk board exposing him to a fall hazard of 7 feet 9 inches to the ground below.  b) At Lot 180 Stone Park, Pike Road, Alabama:  On or about August 16, 2016 and at times prior; the employer allowed his employee to work from a 13 inch metal walk board exposing him to a fall hazard of 15 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At Lot 180 Stone Park, Pike Road, Alabama:  On or about August 16, 2016 and at times prior; the employer allowed his employees to stand on a high slope roof to install siding without fall protection exposing them to a fall hazard of 10 feet to the ground below.  b)  At Lot 180 Stone Park, Pike Road, Alabama:  On or about August 16, 2016 and at times prior; the employer allowed his employees to work from a metal walk board without fall protection exposing them to a fall hazard of 15 feet to the ground below.
