29 CFR 1910.178(l)(4)(ii)(A): Refresher training in relevant topics were not provided to the operator when the operator was observed operating a vehicle in an unsafe manner:  On or about July 5, 2023, refresher training was not provided when an operator was observed performing unsafe operations with a telehandler, thus exposing employees to struck-by or caught between hazards, in the following instances (a-b):  a) Worker sitting on a pallet being lifted to the roof by the telehandler operator; and  b) Operator did not deploy stabilizers as required by the operators manual for the telehandler when lifting workers with a manufacturer's approved lifting device.
