29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about August 2, 2016:  a.  In the Fabrication area where employees were exposed to a struck-by hazard when operating a Press Well notching machine, Model NM80, ID#1, without one or more methods of machine guarding to protect the operator from hazards such as those created by the point of operation, ingoing nip points, rotating parts, and flying chips and sparks.    b.  In the Fabrication area where employees were exposed to a struck-by hazard when operating a Ficep Ironworker Punch, Model TACO 1050 PLC/SP 100, S/N 33544-Punch 3/C, without one or more methods of machine guarding  to protect the operator from hazards such as those created by the point of operation, ingoing nip points, rotating parts, and flying chips and sparks.   JYOTI AMERICAS, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.212(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION 1171293, CITATION NUMBER 1, ITEM NUMBER 2 A AND E AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 24, 2016, WITH A FINAL ABATEMENT DATE OF MARCH 24, 2016, WITH RESPECT TO A WORKPLACE LOCATED AT 3575 POLLOCK DR., CONROE, TX 77303.
