29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.      (a) Production - On or about August 17, 2016, the employer did not ensure that guard rails or equivalent were provided for employees working at elevated heights on Press 62 to prevent employees from falling.     On August 17, 2016, a maintenance employee slipped and fell approximately 23 feet when working on top of Press 62.        In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.

(a) Production - On or about August 17, 2016, the employer did not ensure that employees used the metal portable step ladder as it was intended to be used. When performing servicing, the step-ladder was placed on top of Press 62 and used to reach the switch located above the press. On August 17, 2016, a maintenance employee working on top of Press 62 slipped and fell from approximately 23 feet into the pit.      In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. Chicago Powdered Metal Products Co. was previously cited for a violation of this OSHA Safety and Health standard or its equivalent standard, 29 CFR 1910.22(c) of the Occupational Safety and Health Act of 1970, which was contained in OSHA Inspection Number 1139211, Citation number 1, Item number 1 and was affirmed as a final order on June 15, 2016, with respect to a workplace located at 9700 Waveland Ave, Schiller Park, Illinois 60176.
