29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source:     (a)  Facility, Ranco Volumetric Fertilizer Mixer:  On or about August 10, 2016, the urea screw conveyor was not locked out nor tagged out, nor even shut off when an employee reached up and into the chute.  This employee lost parts of two fingers when a load of urea broke loose and pushed his hand down and into the rotating screw conveyor.  In addition, this machine was not locked out nor tagged out when employees would remove the sliding guard and use an implement to clear a jam in the urea chute.  This approximately 10 by 3 inch sliding guard opening was only about 6 inches from the screw conveyor.
