29 CFR 1910.272(j)(2)(i)(A): The employer's priority housekeeping program did not address floor areas within 35 feet (10.7m) of inside bucket elevator(s):  On or about September 6, 2016, the employer's housekeeping program did not address floor areas within 35 feet (10.7m) of the wood elevator leg at the headhouse level in that dust levels on the floor were greater than 1/8" (.32cm).

29 CFR 1910.307(c): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:  On or about September 6, 2016, electrical equipment in the basement of the feed mill below the roller mill was not approved for Class II combustible dusts.  The list of equipment includes, but is not limited to, electrical disconnects, motor starters, and wiring raceways.
