29 CFR 1910.217(b)(7)(iii): A means of selecting Off, "Inch," Single Stroke, and Continuous (when the continuous function is furnished) shall be supplied with the clutch/brake control to select type of operation of the press. Fixing of selection shall be by means capable of supervision by the employer. On or about September 19, 2016, the employer did not ensure the means of selecting Off, Inch, Single Stroke and Continuous of a Komatsu Model: OBS 60-3, 60 ton part revolution mechanical power press, known as Press #48 was supervised by the employer.

29 CFR 1910.217(f)(2): Instruction to operators. The employer shall train and instruct the operator in the safe method of work before starting work on any operation covered by this section. The employer shall insure by adequate supervision that correct operating procedures are being followed. On or about September 19, 2016, the employer did not insure by instruction and adequate supervision that a temporary employee operating a Komatsu Model: OBS 60-3, 60 ton part revolution mechanical power press, known as Press #48, was aware of and followed the correct operating procedures for the wear and use of hand restraint (holdout) guarding devices.
