29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about December 12, 2024, the employer did not ensure that machine specific procedures for equipment in which employees would perform service and maintenance activities on were developed, documented, and utilized, prior to the work being performed. Employees are tasked with the service and maintenance of equipment including setup of presses without procedures having been developed. Equipment in which these tasks are performed at the facility includes but is not limited to mechanical power presses, press brakes, hydraulic presses, and shears. Hazardous energies present on equipment includes but is not limited to hydraulic, pneumatic, mechanical, gravitational, and electrical.

29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about December 12, 2024, the employer did not ensure that employees tasked with the service and maintenance of equipment, including setup and die changes, had received training as authorized. Employees are tasked with performing tasks which require the use of LO/TO and the isolation of hazardous energies without having been trained as authorized.

29 CFR 1910.217(c)(1)(i):The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es): a) On or about December 12, 2025 the employer did not ensure that effective guarding was in place on the #106 Seyi mechanical power press. Employees operated the press using foot pedal actuation methods at which time the light curtains and barrier guarding did not prevent the employee from reaching the point of operation. Openings in the guarding did not meet the specifications of table O-10 of the standards. b) On or about December 12, 2025 the employer did not ensure that effective guarding was in place on the #107 Seyi mechanical power press. Employees operated the press using foot pedal actuation methods at which time the light curtains and barrier guarding did not prevent the employee from reaching the point of operation. Openings in the guarding did not meet the specifications of table O-10 of the standards. c) On or about December 10, 2025 the employer did not ensure that effective guarding was in place on the #51 Komatsu mechanical power press. Employees operated the press using foot pedal actuation methods at which time the light curtains and barrier guarding did not prevent the employee from reaching the point of operation. Openings in the guarding did not meet the specifications of table O-10 of the standards.
