29 CFR 1910.252(a)(2)(vi)(C): Cutting or welding was permitted in the presence of explosive atmospheres (mixture of flammable gases, vapors, liquids, or dusts with air), or explosive atmospheres that could develop inside uncleaned or improperly prepared tanks or equipment which had previously contained such materials:     A.)  Wm. T. Spaeder Co., Inc., 601 South Street, Saegertown, PA  16433, building #9, on or about September 23, 2016 and times prior thereto:  An employee was tack welding a 2-inch nipple and thread onto the top of a drum.  The drum was not thoroughly inspected and cleaned prior to work operations to eliminate any residual flammable materials in the drum.  The employee was subsequently burned by a flash fire while welding on the drum.

29 CFR 1910.252(a)(3)(i): Welding, cutting, or other hot work was performed on used drums, barrels, tanks, or other containers that had not been cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substances such as greases, tars, acids, or other materials which were subjected to heat, might produce flammable or toxic vapors:    A.)  Wm. T. Spaeder Co., Inc., 601 South Street, Saegertown, PA  16433, building #9, on or about September 23, 2016 and times prior thereto:  An employee was tack welding a 2-inch nipple and thread onto the top of a drum. Spaeder Co., Inc., 601 South Street, Saegertown, PA  16433, building #9, on or about September 23, 2016 and times prior thereto:  An employee was welding on a drum that contained a multitude of chemicals including, but not limited to, MEK, acetone, toluene, and xylene.  The employee was not informed of the specific chemical and physical hazards of the contents within the drum.
