1926.20(b)(2) Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. A.) Wm. T. Spaeder Co., Inc., 451 Fullerton Avenue, Cambridge Springs, PA 16403, boiler / mechanical room area, on or about May 27, 2021 and times prior there to: An employee who was participating in the rigging and lowering of a pipe fell from a ladder when the pipe twisted in the chain of a Harrington 1 Ton hand chain hoist (Code: CF010 / M/N: CF4-0222 / S/N: B003766) and the pipe struck the ladder. The load was improperly rigged in that the hoist chain was wrapped around the load, and that the loop of rope for the Lug-All Wire-Rope Ratchet Winch Hoist (come-along) (M/N: 3000-30) was removed from the load without any additional rigging devices to take its place. B.) Wm. T. Spaeder Co., Inc., 451 Fullerton Avenue, Cambridge Springs, PA 16403, boiler / mechanical room area, on or about May 27, 2021 and times prior there to: Employee were improperly rigging hand chain hoists (chain falls), in that the hoist chain was wrapped around the load. C.) Wm. T. Spaeder Co., Inc., 451 Fullerton Avenue, Cambridge Springs, PA 16403, boiler / mechanical room area, on or about May 27, 2021 and times prior there to: Employee were improperly rigging winch hoists (come-alongs), in that the hoist rope was wrapped around the load. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
