29 CFR 1910.106(e)(9)(i): Maintenance and operating practices did not control leakage and prevent accidental escape of flammable liquids: a. On or about 9/27/16 - AR line - Station 2, at the Sol Mix Applicator, the piping to a branch of the local exhaust ventilation system was disconnected. b. On or about 11/9/16 - AR line -Station 5, at the Sol Mix in line spray applicator, insulation was missing on a branch of the local exhaust system of the unit mounted mist collector. The overspray was not effectively contained.

On or about 9/27/16 - AR upper line, at Station 2, Wypall rags from the Sol Mix spray application were placed in open top plastic containers. b. On or about 9/27/16 - AR upper line, at Station 5, Wypall rags from the Sol Mix spray application were placed in open top plastic containers.
