The employer allowed his employee to work below active overhead work while not wearing any head protection exposing the employee to head injuries from falling materials and or tools. 29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:  a) Building 43, Harbortown Blvd., Perth Amboy, NJ 08861. The employer allowed his employee to operate a pneumatic nail gun to secure roof sheathing while not wearing any eye and or face protection exposing the employee to eye and face injuries from flying debris.    Occurred on or about 10/05/16.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) Building 43, Harbortown Blvd., Perth Amboy, NJ 08861. The employer allowed his employee to secure roof sheathing on a three story apartment complex with no fall protection system in place exposing the employee to fall hazards in excess of 30 feet.     Occurred on or about 10/05/16.
