29 CFR 1917.95(c): The employer did not provide well maintained emergency bathing, eye washing or other facilities where employees were exposed to hazardous substances.   Maintenance Shop Area:  On or about 25 Oct 2016 and times prior thereto, the wall mounted, Serian eye wash station was blocked with supplies and equipment.  Employees servicing vehicles from 55 gallon barrels of motor oil and ant-freeze/Coolant on a routine basis.  Eye wash station was last serviced/inspected on May 16, 2012. This condition exposed delays in employees to utilize the eyewash.

29 CFR 1910.1200(g)(8): The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace.  (a) Machine Shop:  On or about 25 October 2016 and times prior thereto, MSDS/SDS were not available for All Season Windshield Cleaner-Deicer.  This condition exposed employees to contact with toxic, caustic or noxious substances.    (b) Machine Shop:  On or about 25 October 2016 and times prior thereto, MSDS/SDS were not available for Harbor Master Cleaner.  This condition exposed employees to contact with toxic, caustic or noxious substances.   Samson Tug and Barge, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.1200(g)(8), which was contained in OSHA inspection number 1020552, citation number 1, item number 2 and was affirmed as a final order on 4/8/2015, with respect to a workplace located at the Samson Terminal, 102 Outer Drive, Wrangell, Alaska.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein   Maintenance shop:  On or about 24 October 2016 and times prior thereto, unattended plastic bottle; containing an unknown chemical substance was not marked identifying the product and the hazard warnings associated with it.  This condition exposed employees to chemical burns, poisoning, and skin dermatitis.
