OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  Warehouse; on or about 25 October 2016 and times prior thereto; two warehouse storage racks were not secured to the wall or floor. Storage racks did not have the maximum safe load limits posted. Forklifts are used to place items on storage racks. This condition exposed employees to a struck by hazard.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.  Warehouse; on or about 25 October 2016 and times prior thereto; four instances were noted where energized electrical openings were not effectively closed:       a. Bay 1, adjacent to roll-up door; a knock out plug missing on lower left side of a energized light switch.       b. Bay 6, a knock out plug missing on top right hand side of a two place energized electrical outlet.       c. Bay 5, a two place energized electrical outlet was damaged and partially attached.            d. Backroom inside wall, a two place energized electrical outlet had a damaged faceplate.   This condition exposed employees to electrical shock and electrical burn hazard.
