29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:      a) On or about 11/3/2016, in the mixing room and on the production floor, copper dust and other chemical powder materials used in the production process has accumulated on walking and working areas. The employee is exposed to slips, trips, and falls.

29 CFR 1910.212(a)(2): Machine guard(s) on machines were so installed that they offered accidental hazards in themselves:    a) On or about 11/22/2016, in the mixing room the employer selected the wrong type of clasp for the guarding mechanism.  The machine guard protects the employee from the mixer while in operation.  When mixing is complete the employee lifts the guard which is attached to two pulleys via a clasp in order to enter the mixing area and empty the contents of mixer into a drum.  Several clasps were observed to be failing.
