29 CFR 1910.212(a)(2): Machine guard(s) on machines were so installed that they offered accidental hazards in themselves:    a) On or about 11/22/2016, in the mixing room the employer selected the wrong type of clasp for the guarding mechanism.  The machine guard protects the employee from the mixer while in operation.  When mixing is complete the employee lifts the guard which is attached to two pulleys via a clasp in order to enter the mixing area and empty the contents of mixer into a drum.  Several clasps were observed to be failing.
