29 CFR 1910.22(a)(3): Every floor, working place, and passageway was not kept free from protruding nails, splinters, holes, or loose boards: Lorain County Automotive Systems, Inc. Lorain, Ohio; on or about December 1, 2016 the employer did not ensure that the walking working surface in the Camaco Inspection Area was maintained free of holes. Employees were exposed to trip and fall hazards in that drainage grates had been removed or damaged leaving gaps in the floor measuring approximately five inches wide, five inches deep, and six feet long in areas.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 ppm: Instance A On or about November 23, 2016, an employee working in the testing area was overexposed to carbon monoxide at an eight hour time weighted average of approximately 151.4 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by approximately 302.8%. Instance B On or about November 23, 2016, an employee working in the assembly area was overexposed to carbon monoxide at an eight hour time weighted average of approximately 101.4 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by approximately 202.8%. Instance C On or about November 23, 2016, an employee working in the shipping and receiving area was overexposed to carbon monoxide at an eight hour time weighted average of approximately 116.7 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by approximately 233.4%.

An employee working in the testing area was overexposed to carbon monoxide at an eight hour time weighted average of approximately 151.4 ppm, which exceeded the OSHA permissible exposure limit of 50 ppm by approximately 302.8%. b) Lorain County Automotive Systems, Inc.
