29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a) Wheel Polishing Area - Employees are exposed to electrical hazards since the junction box leading to the ventilation fan located on the floor behind the wheel polisher did not have a cover on it.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: a) Wheel Polishing Area - Employees working in the wheel polishing area are provided with tight fitting respirators to use on a volunteer basis while working in the tire buffing area to protect against dust generated by the process and were not provided the information found in Appendix D of the respiratory protection standard.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: a) Wheel Polishing Area - Employees are provided tight fitting half face respirators to use on a volunteer basis while polishing tires to protect against the dust generated by the process and the employer did develop and implement the elements of a written program to insure they are medically able to wear them.
